EL Approval Form

You must submit this form before you begin your experiential learning opportunity. In order to complete the form, you’ll have had to meet with someone from your Pathways faculty/staff team to plan out your experience and make sure it’ll meet the requirements of the Pathway.

Contents of the Form – Please Review

Please review the form before you start working on it in Forms, as it requires information from your experience host/employer. Notice that this form contains “branching,” so the middle section contains some portions that will be irrelevant to you. Here are the most important pages to review, depending on the type of experience you’re completing:

• EL-credentialed course or TREK – page 4
• On-campus job, activity, or research experience – page 5-6
• Experience during study away (GEO) – pages 7-9
• Off-campus opportunity besides study away – pages 10-11
