Career Readiness Assessment Form – Supervisor/Coworker

Pathways students will need to request that a current or former supervisor or coworker complete this form on their behalf. Students compare their supervisor/coworker’s thoughts with their own self-assessment and spend some time reflecting on similarities and differences and what can be learned from them.

Who can complete this form?

  • Choose someone you’ve worked for or with who would have a strong sense for your career readiness.
  • It could be a manager, coach, neighbor you’ve done work for, adult coworker, club supervisor, volunteer supervisor, or so on.
  • As with any professional reference or evaluation, it must not be filled out by a family member or close friend in order to gain a more unbiased assessment.

Before giving the form to your supervisor/coworker:

  • Fill in the Student fields at the top
  • Mark your 3-4 professional development focus areas. Please review this document to determine your focus areas if you haven’t done so yet. It is recommended that you consider these factors as you determine your as focus areas:
    • At least one you believe is a weakness and are working to grow in
    • At least one you believe to be a strength so you can see if your self-assessment matches what your supervisor saw
    • Ones that are particularly crucial in your field of interest (noting that all eight are important in every 21st century career). For example, technology for a data analysis job or communication for a marketing position.
  • Send it to your supervisor/coworker, asking them to return it to you by a certain date. We recommend giving them 2-3 weeks regardless of when you actually need it so they are more likely to remember to complete it.

After you get it back from your supervisor/coworker:

  • Review it alongside your own self-assessment of these eight career competencies to reflect on your strengths and weaknesses, making observations and using them to plan for future growth, learning, and behavior. You will find specific guidance for this reflection in the Moodle.
